Petite Fleur's team of caring social workers provides psychosocial and emotional support, case management, advocacy, and assistance with financial matters. We offer residents and families the support and guidance needed during all stages of a resident's stay; the admission process, applying for Medicaid, social adjustment and facilitation, discharge planning, etc. Social workers are available to meet the individual needs of our residents and to address family concerns.

     If a resident's stay is temporary, discharge planning includes coordination of all needed health care services, such as physicians, social workers, home care nurses, rehabilitation therapists and pharmacy. This interdisciplinary approach helps ensure a safe and comfortable transition when returning home.
